Delivery of care to patients is based on criteria that are specific to the physical, psychosocial, cultural, safety and learning needs of that individual, depending on age and developmental level and on competence of staff assigned to that department. 

  

 The CNC reviews patient care daily, visits patients regularly, making suggestions to improve care and becomes involved in the care of selected patients.  The CNC organizes, coordinates, and facilitates patient care on her/his department/unit during all operating hours.  The CNC performs delegated management duties, assigned by the Patient Care Director, which are consistent with her/his scope of nursing practice and competence.  The CNC demonstrates an understanding of the budget process and staffing plan, implementing cost-effective strategies in staffing and supply management decisions.  She/he participates in the interviewing, monitoring, coaching, evaluating and disciplining process.  The CNC supports the mission, vision, and values, goals and objectives of Presbyterian/St. Luke's Medical Center and the HealthONE organization.

  

 The scope and complexity of the CNC position will vary based on the number of beds/visits/procedures in the department, acuity of patients, FTEs supervised, number of different medical staff and medical staff services involved, "normal working hours" responsibility or 24/7 responsibility, the activity level of admissions, discharges and transfers per day, and the education resources available.

 The ICU/CCU RN manages the nursing care of critically ill patients in accordance with established policies, procedures and protocols of the healthcare organization.  The tasks and responsibilities include:

 
·Assesses plans and evaluates patient care needs. 

·Carries out physician orders.

·Administers prescribed medications, changes dressings, cleans wounds, monitors vital signs.

·Provides respiratory and ventilator management and monitoring for patients.

·Serves as the primary coordinator of all disciplines for well coordinated patient care.

·Monitors, records and communicates patient condition as appropriate utilizing computerized documentation systems. 

·Instructs and educates patients and families.

·Assesses and coordinates patient's discharge planning needs with members of the healthcare team.

·Provides age and culturally appropriate care.

·Orients and mentors new staff members.

·Follows Standard Precautions using personal protective equipment as required.

 The NICU II RN provides direct nursing care to neonates and their families in accordance with established policies, procedures and protocols of the healthcare organization.  The tasks and responsibilities include:

 
·Assesses, plans and implements care and interventions in addressing the problems of the neonate.

·Carries out physician orders.

·Assists physician during examination, treatment, and procedures.

·Administers prescribed medications, changes dressings, cleans wounds, monitors vital signs.

·Provides nutritional assessment and care to the infant.

·Promotes family centered nursing care for neonates.

·Serves as the primary coordinator of all disciplines for well coordinated patient care.

·Monitors, records and communicates patient condition as appropriate utilizing computerized documentation systems. 

·Instructs and educates patients and families.

·Assesses and coordinates patient's discharge planning needs with members of the healthcare team.

·Provides age and culturally appropriate care.

·Maintains a safe and therapeutic patient care environment for at risk infants.

·Orients and mentors new staff members.

·Follows Standard Precautions using personal protective equipment as required.
