 Position Summary:   This position supports the mission of Presbyterian/St Luke's Medical Center of optimizing the quality of life of all those served by expanding beyond the traditional focus on medical needs to the needs of the whole person.  The   Social Worker III   is responsible for assessing the psychological, social, cultural and/or financial needs that impact the high-risk patient's health and recovery.  Has the ability to utilize social services, counseling and clinical knowledge in the assessment of these needs.  Responsible and accountable for coordination of services through an interdisciplinary process which provides a clinical and psychosocial approach through the continuum of care and balances individually identified patients and family needs with the efficacious and cost effective utilization of resources.  Through concurrent case management, patients will be assessed to determine appropriateness of admission, continued hospitalization, as well as appropriate level of care.  Discharge planning will begin at the time of (or prior to) admission, and reassessed ongoing throughout the course of hospitalization.  Knowledge of payer source implications on discharge plan, resources and placement options to maximize patients successful disposition.

  I. Position Summary:  The Medical Social Worker II is responsible and accountable for assessing the psychological, social, emotional, cultural and/or financial needs that impact patients' health and recovery. The MSW II shall:

 · Utilize social service, supportive counseling, and clinical knowledge in the assessment of these needs

 · Address assessed needs in a timely manner

 · Develop a discharge plan in collaboration with the patient/family and the interdisciplinary team

 · Utilize knowledge of payer source implications on discharge plan, resources and placement options to maximize patients' successful disposition

 · Perform delegated duties consistent with the scope of practice and functions within the philosophy and overall plan of care at NSMC

 · Serve as a resource for issues of financial assistance and/or resources that promote continued independence for patients

 · Assist patients and families in matters related to advance directives and guardianship and proxy decision making

 · Perform psychiatric evaluations for patients on mental health, drug and/or alcohol holds or access contracted professionals to do so

 · Provide short term counseling in areas of grief, loss, anxiety and stress related to illness
